Case Challenge #3

  • A 19 year old white male is admitted with acute onset fever and abdominal pain. Multiple recurrences over last 10 years. Episodes resolve in 3-4 days. Otherwise, surgical history is significant for an appendectoy done at age 16. No significant social or family history.
  • PE: Tmax 102.2, peritoneal signs on exam.
  • Labs: WBC 17k, Hgb 12. Other labs wnl. FOBT negative, CT A/P unremarkable.
